Always there in times of need, our client provides support and assistance to those who require assistance in dealing with some of life’s tougher issues. We are looking for an experienced and qualified Emergency Relief Case Manager to provide much needed support and advocacy for these vulnerable people.
You will be responsible for developing individual case plans with clients and regularly monitoring and reviewing these plans to ensure they meet the client’s goals, liaising with other agencies and government departments, providing basic counselling to clients, as well as assistance and access to community support services and reporting on the progress and outcomes of the case plans. You will liaise with other service providers for referral contacts, attend regular team meetings and identify any gaps and make recommendations for service delivery improvements.
You will work well within a small team setting, have previous experience in a similar role within the social service environment and have a Diploma in Community Services or higher. A National Police record check, Working with Children and NT Drivers licence are also a requirement for this role, as is a committed, empathetic and client focussed approach. This position also has the opportunity for permanency for the right candidate.
